Referrals

We appreciate your trust in our services and the opportunity to support individuals with disabilities on their NDIS journey. Referrals are a valuable way to connect individuals with the right support and empower them to achieve their goals.
By referring a participant to us, you are helping us make a positive impact in the lives of individuals with disabilities. We appreciate your trust and collaboration in supporting individuals on their journey towards greater independence and well-being.
To refer a participant please follow the steps below:
- Obtain Consent: Before referring a participant, ensure that you have obtained their consent and explained the purpose and nature of the referral process. Respecting the participant’s autonomy and confidentiality is of utmost importance.
- Gather Information: Gather relevant information about the participant, such as their NDIS plan, support needs, goals, and preferences. The more information you can provide, the better we can tailor our services to meet their individual requirements.
- Complete the Referral Form: Please fill out the referral form provided at the bottom of this page accurately and provide as much detail as possible. This will enable us to understand the participant’s needs and goals effectively. Our intake team will review the referral and reach out to the participant as appropriate.
We accept referrals from various sources, including:
- Individuals and Families: Participants and their families can directly refer themselves to Supported Living Australia for NDIS support services. We understand that individuals and families are often best placed to identify their needs and preferences.
- Support Coordinators and Allied Health Professionals: Support Coordinators and Allied Health Professionals: Support coordinators, allied health professionals, and other service providers who work closely with individuals with disabilities can refer participants to us. These professionals play a vital role in assisting participants to access the support services that align with their goals and needs.
